Dept. of Revenue: New retail license form for cigarette, tobacco product and electronic vaping device retailers now available  

MADISON — The new retail license form for retailers of cigarettes, tobacco products and electronic vaping devices, Form CTV-100, is now available from the Wisconsin Department of Revenue.
