MADISON — The new retail license form for retailers of cigarettes, tobacco products and electronic vaping devices, Form CTV-100, is now available from the Wisconsin Department of Revenue. This form replaces the prior form, Form CTP-200, which is a retail license to sell cigarette and tobacco products.
