Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Marian Aronson, 791 Park of Commerce Blvd., Ste. 201, Boca Raton, FL 33487-3634, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of using another insurance agent’s NPN to enroll a consumer in a health insurance plan and failing to respond to an inquiry from OCI.

Joseph T. Bubloni, 418 Birchwood Dr., Antioch, IL 60002, had his insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Summer M. Crawley, 8136 S. Mission Dr., Franklin, WI 53132-2353, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Emma Gabor, 4120 S. Lake Dr., Unit #364, St. Francis, WI 53235, had her application for an insurance intermediary license denied for 30 days.  This action was taken based on allegations of using fraudulent, dishonest practices and demonstrating financial irresponsibility in the conduct of business.

Chris J. Gaffney, 20250 Merry Oak Ave., Tampa, FL 33647-3642, had his non-resident insurance license revoked. This action was taken based on allegations of violating insurance laws and lacking the character required of insurance intermediaries.

Jeimmy Garces, 9603 NW 36th Mnr., Coral Springs, FL 33065-2868, had her insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due, failing to respond, and other insurance violations.

Blake Hemmersbach, 408 Academy St., Apt. 3, Elroy, WI 53929, had his application for an insurance intermediary license denied.  This action was taken based on allegations of making a material misrepresentation in the information in his license application and failing to disclose criminal charges.

Roxanne Hendon, 3318 N. 38th St., Milwaukee, WI 53216, had her application for an insurance intermediary license denied.  This action was taken based on allegations of demonstrating financial irresponsibility in the conduct of business in this state or elsewhere.

Rafael Hidalgo, 6203 Williamsburg Way, Apt. 114, Deforest, WI 53532-9121, had his insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Joseph A. Hill, 4424 E. Baseline Rd., Apt. 1001, Phoenix, AZ 85042, had his insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Shannon Hilliard, 2911 N. 91st St., Milwaukee, WI 53222-4618, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Quantia Johnson, 3409 Rosecliff Trce., Buford, GA 30519-3021, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Blake Levy, 17029 Wandering Wave Ave., Boca Raton, FL 33496, had his application for an insurance license denied.  This action was taken based on allegations of having violated an insurance law or regulation and having administrative actions taken by other states and the Financial Industry Regulatory Authority.

Maurice J. Lindquist, N58W25600 Windy Pass Dr., Sussex, WI 53089-4133, had his insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Alejandro Morales, 349 S. Central Ave., Unit B, Marshfield, WI 54449, had his application for an insurance license denied.  This action was taken based on allegations of having criminal convictions substantially related to the circumstances of holding an insurance license and providing incorrect, misleading, or incomplete information in the license application.

Coleman A. Pollock, 140 Cypress Station Dr., Ste. 230, Houston, TX 77090-1633, was ordered to pay a forfeiture of $500.00. This action was taken based on an allegation that he authorized the use of his NPN by another insurance agent to enroll a consumer in a health insurance plan.

Anthony Redmond III, 3141 Fish Hatchery Rd., Fitchburg, WI 53713-3126, agreed to the permanent revocation of his Wisconsin insurance license and a restitution forfeiture of $97,549.26.  This action was taken based on allegations of premium misappropriation and misrepresentations.

Irma M. Ruiz Villanueva, 18117 Biscayne Blvd., Ste. 1584, Aventura, FL 33160-2535, agreed to the revocation of her Wisconsin non-resident insurance license. This action was taken based on allegations of lacking the character required of insurance intermediaries.

Dominic Sanchez, 2641 Milwaukee St., Madison, WI 53704, had his insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Nicholas Sims, 4605 N. 107th St., Wauwatosa, WI 53225, had his insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Julian Smith, 62 W. Cheltenham Ave., Philadelphia, PA 19120, had his application for an insurance intermediary license denied.  This action was taken based on allegations of having a criminal conviction substantially related to insurance marketing conduct.

Carolyn Turner, 3858 Washington Ave., #18, Cincinnati, OH 45229, agreed to the voluntary surrender of her public adjuster registration.  This action was taken based on allegations of failing to maintain an active public adjuster bond in favor of the State of Wisconsin and failing to respond to inquiries from OCI.

Alexander R. Wells, 2559 Hall Johnson Rd., Apt. 1318, Grapevine, TX 76051, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of using another insurance agent’s NPN to enroll a consumer in a health insurance plan, failing to respond to an inquiry from OCI, and failing to provide an updated mailing address to OCI.

Insureme Inc., 842 Hamilton St., Ste. 800, Allentown, PA 18101, agreed to pay a forfeiture of $500.00.  This action was taken based on allegations of failing to disclose an administrative action taken by another state.

Aliezka Hernandez, 809 E. Main St., Unit #1, Waukesha, WI 53186, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence that she committed a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

Simran Karki, 1042 N. High Point Rd., Madison, WI 53717, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence that he committed a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

Rebecca Lancaster, 16802 Hamill Dr., Rosharon, TX 77583, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence that she committed a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
