Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Jaylah D. Askew, 15511 Jessie Parker Rd., Sugar Land, TX 77498, was ordered to pay a forfeiture of $2,000.00. This action was taken based on allegations of failing to timely report to OCI an administrative action taken by the State of Indiana and failing to respond to an investigative demand.

Katrina N. Avery, 11127 Barker West Dr., Cypress, TX 77433, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Bradley Bahan, 3362 Garden Bar Rd., Lincoln, CA 95648-9752, agreed to pay a forfeiture of $500.00. This action was taken based on allegations of borrowed NPN laws/rules.

Alexander Cook, 5930 Coral Lake Dr., Margate, FL 33063-5853, had his insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due, failing to respond, and other insurance violations.

Frank Crivello, 2385 Northside Dr., Ste. 200, San Diego, CA 92108-2702, agreed to pay a forfeiture of $500.00. This action was taken based on allegations of borrowed NPN laws/rules.

Robert Dixon, 2545 S. 44th St., Apt. 22, Milwaukee, WI 53220, had his insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Patricia A. Ewer, 624 130th St., Amery, WI 54001, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Ava D. Ford, 7626 W. Knightsbridge Dr., Mequon, WI 53097, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Enmanuel Garcia Colon, 872 Smithfield Ave., Lincoln, RI 02865-3551, agreed to a voluntary surrender of his non-resident Wisconsin insurance license.

Grayson D. Hill-Boll, 717 Churchill St., Waupaca, WI 54981, had his insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Jasmine Jasso, 175 Berkeley St., Boston, MA 02116, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Stephanie Michel, 2783 Elmwood Rd., Suamico, WI 54313, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Omar Mitchell, 3538 Creek Meadows Dr., Pickerington, OH 43147, agreed to the voluntary surrender of his insurance intermediary license.  This action was taken based on allegations of failing to respond to inquiries by OCI.

Fabio Muhameti, 310 E. South Blvd., Troy, MI 48085, agreed to permanently surrender his insurance intermediary license.  This action was taken based on allegations of making misrepresentations in connection with insurance solicitations and sales.

Traci S. Onsgard, PO Box 62, La Crosse, WI 54602-0062, had her insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due, failing to respond, and other insurance violations.

Scott Pederstuen, 924 Center St., Boyceville, WI 54725, had his insurance license revoked. This action is based on allegations of failure to pay a previous forfeiture when due and failure to respond.

Frederick Phelan, 6230 Pebblebrook Ct., Lincoln, NE 68516, agreed to a one-year probationary period on his insurance license.  This action was taken based on allegations of having an administrative action taken by the Financial Industry Regulatory Authority.

Lakeesha Robinson, 3801 Southern St., Little Rock, AR 72206, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Rosibel Solis Gomez, 9668 W. Arch Ave., Milwaukee, WI 53224, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.
