Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Saafir J. Bellamy, 3921 N. Humboldt Blvd., Apt. 401, Milwaukee, WI 53212, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Derek M. Bennett, 5728 Seton Dr., Margate, FL 33063, had his application for an insurance intermediary license denied.  This action was taken based on allegations of having insurance licenses suspended and revoked in other states and having administrative actions taken by other states.

Teisha N. Blonvia, 3801 6th Ave., South Milwaukee, WI 53172, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Sherry Booker, 4239 N. 68th St., Milwaukee, WI 53218, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Yuemei Ding, 845 Indian Rd., Glenview, IL 60025-3313, had her insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due.

John Jackson, 211 Cleveland Ave., Harrogate, TN 37752, was ordered to pay a forfeiture of $500.00.  This action was taken based on allegations of failing to timely disclose an administrative action taken by another state and failing to respond to inquiries by OCI.

Dana M. Jensen, P.O. Box 725, Deerfield Beach, FL 33443, had his insurance license revoked.  This action was taken based on allegations of failing to timely report administrative actions taken by other states and having a license revoked by another state based on allegations of misrepresentation on an insurance application.

Kyle T. Johnson Cherek, 3916 N. Oakland Ave., Shorewood, WI 53211, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Toni D. Jones, 3740 W. Clinton Ave., Milwaukee, WI 53209, had her application for an insurance intermediary license denied.  This action was taken based on allegations of making material misrepresentations and providing incomplete information on her license application.

Jon K. Lacasse, 14502 30th St. N., Stillwater, MN 55082, had his application for an insurance intermediary license denied.  This action was taken based on allegations of failing to disclose administrative actions, having regulatory actions taken with regard to an occupational license, and violating an order issued by the insurance commissioner.

Jennifer M. Loesch, 108 W. Grove St., Burlington, WI 53105, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Victoria Love, 2211 Montgomery Park Blvd., Apt. 125, Conroe, TX 77304, had her insurance license revoked. This action was taken based on allegations of multiple violations of insurance statutes and regulations.

Michael Lyons, 700 8th St. N., Apt. H, Hudson, WI 54016, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Matthew A. Malone, 6222 Tranquil River Ln., Wausau, WI 54401, had his application for an insurance intermediary license denied.  This action was taken based on allegations of failing to disclose administrative actions taken by other states and failing to pay Wisconsin state taxes.

Anthony Redmond III, 3141 Fish Hatchery Rd., Fitchburg, WI 53713, had his insurance license summarily suspended.  This action was taken based on allegations of misappropriating insurance premiums from customers, misrepresenting refunds of premiums, and demonstrating untrustworthiness in the conduct of business in this state.

Tammy Richardson, P.O. Box 601, Montello, WI 53949, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Nathaniel Robinson, 3736 Valley Bluff Ln., Snellville, GA 30039-2512, had his insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due and other violations.

Jason Young, 2000 Galahad Dr., Deltona, FL 32738, was ordered to pay a forfeiture of $500.00.  This action is based on allegations of failing to report an administrative action taken by another state.

Vault Business Group LLC, 603 Manor Ct., Altoona, WI 54720, had their insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due and other violations.

Wathen Financial Group Gen Ptr, 5975 N. State Rd. 236, North Salem, IN 46165, agreed to the voluntary surrender of its intermediary’s license.  This action is dismissed without prejudice.

Allegations and Actions Against Other Entities

Shannon Clary, 305 Frost Woods Rd., Monona, WI 53716, was ordered to pay a forfeiture of $100.00.  This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

Kenneth Compton, 1460 S. Carriage Ln., Apt. 8, New Berlin, WI 53151, was ordered to pay a forfeiture of $100.00.  This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

Larry Haynes, 2009 Northport Dr., #15, Madison, WI 53704, was ordered to pay a forfeiture of $100.00.  This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

Cameron Jones, 1866 Fisher St., Apt. 1, Madison, WI 53713, was ordered to pay a forfeiture of $100.00.  This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
