Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Kevion Dean-White, 9560 N. 60th St., Brown Deer, WI 53223, had his application for an insurance intermediary license denied.  This action was taken based on allegations of having and failing to disclose criminal matters substantially related to the circumstances of holding an insurance license.

Cuauhtli Esparza, 1325 Mendota St., Madison, WI 53714, had his application for an insurance intermediary license denied.  This action was taken based on allegations of having criminal matters substantially related to the circumstances of holding an insurance license.

Aaron Fox, 1200 N. Federal Hwy., Ste. 315, Boca Raton, FL 33432-2846, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product.

Drew Foxcroft, 1417 Willmor St., Racine, WI 53402, had his application for an insurance intermediary license denied.  This action was taken based on allegations of having criminal convictions substantially related to the circumstances of holding an insurance license.

Joseph B. Griffin-Hubbard, 5120 Brookeside Dr., Suite 203, Madison, WI 53718, had his insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Brentell Handley, 1717 Park St., Naperville, IL 60563, had his application for an insurance license denied for 75 days.  This action was taken based on allegations of having a criminal matter substantially related to the circumstances of holding an insurance license.

Bruce J. Hardy, 1425 Trumpet Lily Pl., Nolensville, TN 37135, had his application for an insurance intermediary license denied.  This action was taken based on allegations of having a criminal conviction substantially related to the circumstances of holding an insurance license.

Brenda Hermansen, 994 206th Ave., Luck, WI 54853-2305, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Ashlynn Hughes, 19506 Star Haven Dr., Cypress, TX 77433, was ordered to pay a forfeiture of $1,500.00.  This action was taken based on allegations of failing to respond to inquiries from OCI.

Seth Keturakat, Liberty Mutual, Boston, MA 02116, had his application for an insurance intermediary license denied.  This action was taken based on allegations of having criminal convictions substantially related to the circumstances of holding an insurance license and having administrative actions taken by other states.

Alexander Kim, 24240 Genesee Village Rd., Golden, CO 80401-9381, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product.

David M. Mogensen, 427 Saratoga Ave., North Fond Du Lac, WI 54937, had his insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Iris Ortiz, 2954 N. 36th St., Milwaukee, WI 53210-1926, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

John Pelletier, BMO Investment Services, 7501 W. North Ave., Wauwatosa, WI 53213, agreed to pay a forfeiture of $1,000.00 and agreed to a probationary license period of one year.  This action was taken based on allegations of having and failing to timely report an administrative action taken by the Financial Industry Regulatory Authority, and failing to maintain accurate contact information.

Latonya L. Quin, 185 Woodward Ln., Jackson, GA 30233, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

David Ramos Viana, 5562 NW 41st Ave., Coconut Creek, FL 33073-4020, agreed to the revocation of his Wisconsin non-resident insurance license. This action was taken based on allegations of engaging in unfair marketing practices, including improperly enrolling multiple Wisconsin consumers into health insurance plans without their authorization, consent, and knowledge.

Matthew Ricci, 260 1st Ave. S, Ste. 200, Saint Petersburg, FL 33701-4364, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of authorizing the use of his NPN by another insurance agent to enroll a consumer in a health insurance plan.

Steven Sichel, 482 Burton Ave., Highland Park, IL 60035, agreed to pay a forfeiture of $500.00.  This action was taken based on allegations of failing to disclose an administrative action taken by another state.

Justin W. Stenberg, 1502 Greenfield St., Apt. 910, Baraboo, WI 53913-2824, had his insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Eric Tartaglione, 506 Academy Ave., Staten Island, NY 10307, had his application for an insurance license denied.  This action was taken based on allegations of having regulatory actions in other states that may have been for fraudulent, coercive, or dishonest practices in the conduct of business. 

Mahogany M. Warmack, 3041 S 56th St., Apt. 50, Milwaukee, WI 53219-3120, had her insurance intermediary license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Tania Wiedenhoeft, 8734 W. Fond Du Lac Ave., Milwaukee, WI 53225, had her application for an insurance license denied.  This action was taken based on allegations of having a criminal conviction, which may be substantially related to insurance. 

Robert A. Williams, 10627 Tall Oak Dr., Fort Worth, TX 76108-3755, was ordered to pay $16,000.00.  This action was taken based on allegations of multiple counts of selling unauthorized health insurance to Wisconsin consumers, providing Wisconsin consumers false and/or misleading information relating to insurance contracts, and failing to respond to OCI investigation demands.

Charming Wurm, 6424 NW 5th Way, Fort Lauderdale, FL 33309-6112, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product.

Miguel Torrijos, 6224 W. Mitchell St., West Allis, WI 53214, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

Natasha Wright, 5862 N. 38th St., Milwaukee, WI 53209, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
